Hazard and Incident Reporting

We have an obligation to provide a work environment without risks to health and safety, so far as is reasonably practicable. Our Procedure requires hazards and incidents to be reported, investigated, controlled and monitored.

Report a Hazard

A hazard is a situation or thing that has the potential to harm a person.

Report an Incident

An incident is an unplanned event related to a person resulting in or potential for injury/illness or other loss. This includes work-related injury/illness, property damage, near miss, theft or a security incident.

Report a Confidential Incident

A confidential incident allows a person to report an incident in confidence. Examples include harassment, misconduct, sexual violence, critical incidents, or distress.
