These forms are for seeking permission to use third-party copyright material from the copyright owner. Once you have received written permission, you must lodge a copy with the Library's Course Resources Online (CRO) team for recordkeeping.

These notices must be prominently displayed whenever a copyright work is copied or communicated (including emails) to students or colleagues.
Use this label with video-streaming made for Online Media only under Part VA and/or publisher's permissions.
- A4 [pdf] displayed on all photocopies or scans of book chapters, journal articles, internet pages, artistic works, notated music, etc, or whenever you communicate copyright works online eg. email, or put on Moodle) under the CAL Licence Agreement .
These forms are required if you are taking CD-ready or print-ready materials to EDT for the production of course packs/readers (print or CD):

The wording for these notices, which must be reproduced as an A4 size document, is found in Schedules 3 and 9 of the Copyright Amendment Regulations 2010 (No. 1) .
There are two notices: 2011-warning-MFD39A-104B (notice 1 ); 2011-warning-audiovisual104B (notice 2 ).
The notices for "libraries" and archives should also be placed near recording and copying equipment provided by an organisation in areas other than their library or archive, such as in staffrooms and laboratories.

The notices will generally limit the university's liability for copyright infringement when copying and recording equipment is used by people outside the organisation or by students. The organisation may still, however, be vicariously liable if its staff use the equipment to infringe copyright.
Please make sure that students and staff can see the notices when they use the equipment, reminding them of their copyright obligations.
