Please contact your Rehabilitation Coordinator or alternatively the Health, Safety & Wellbeing Unit on 4930 6418.

You may find CQU's Employment Assistance Program useful. Follow the link for more in depth information. Briefly this is a service offered by CQUniversity to provide various forms of counselling eg financial and phsycological, to both you and you family members. It is confidential, numbers are reported to CQUniversity names of persons utilising the service are not.

Rehabilitation Kit

You must obtain a medical certificate from your doctor.
Work Capabilities Checklist (to be completed by your doctor if you are returning to work, this form can substitute for a medical certificate for non-work related illness/injury)
Application for Leave - via ESSO or Form
